Who we are
The Community HealthCare Association of the Dakotas (CHAD) is a non-profit membership organization that serves as the primary care association for North Dakota and South Dakota. At CHAD, we believe that everyone has a right to high-quality, reliable, affordable health care, regardless of where they live. We work with health centers, community leaders, and partners to increase access to and improve health care services in areas of the Dakotas that need it most
Building high-quality, accessible care for all
For over 40 years, CHAD has supported health centers through technical assistance, education, and advocacy. Alongside Great Plains Health Data Network (GPHDN) and Great Plains Clinically Integrated Network (GPCIN), we provide a comprehensive support ecosystem across North Dakota, South Dakota, and Wyoming.
Community, Tribal, and Urban Indian health centers deliver integrated medical, dental, and behavioral care to all individuals, regardless of insurance or ability to pay. Located in rural, frontier, and urban areas, they ensure access to care for all communities.

Great Plains Clinically Integrated Network
Great Plains Health Data Network
The GPHDN helps health centers leverage health information technology (HIT) and data to enhance delivery of affordable, accessible, and high-quality primary health care. Together, members focus on data management and analytics, interoperability of systems, population health management, and digital health tools such as telehealth, remote patient monitoring, and electronic messaging. The GPHDN’s 11 participating health centers serve a combined 103,142 patients across North Dakota, South Dakota, and Wyoming.
Get Covered South Dakota
Get Covered South Dakota is a statewide Navigator program led by CHAD. We provide free, one-on-one support to help South Dakotans understand their health insurance options and enroll in Marketplace or Medicaid plans.
What is a health center?
Community health centers and Tribal and Urban Indian health centers provide comprehensive, integrated primary, dental, and behavioral health care to more than 162,600 individuals at 66 sites in 50 communities across North Dakota and South Dakota.
Health centers provide care to all people, regardless of insurance status or ability to pay.

Members support
CHAD’s team of experts provides health center members with resources to support key areas of operations, including clinical, human resources, data, finance, compliance, outreach and enabling, and communications. This includes facilitation of network teams, where health center staff can share best practices, learn from one another, and gain access to key tools and resources.
Clinical quality
Clinical services require ongoing education to maintain compliance, achieve accreditation, and support continuous quality improvement. CHAD helps health centers identify best practices, innovative programs, curricula, and funding opportunities to enhance operations and expand integrated care models.
