We are thrilled to announce the launch of the Great Plains Clinically Integrated Network (GPCIN), a new regional collaboration to support rural health care access, increase operational efficiency, and advance value-based care models.
The GPCIN consists of 15 federally qualified health centers (FQHCs), collectively serving more than 162,000 patients with a footprint that spans North Dakota, South Dakota, Wyoming, and neighboring states.
Together, we’re building a high-quality, accessible system of care for all.
Health centers, or FQHCs, are local, nonprofit providers dedicated to delivering comprehensive, integrated primary medical, dental, and behavioral health care to all individuals, regardless of insurance status or ability to pay. Located in rural, frontier, and urban communities, health centers play a critical role in sustaining access to care for rural and underserved communities. Learn more about local health centers.
Through a flexible, opt-in model, the GPCIN creates a platform for health centers to collaborate on value-based care, shared services, group purchasing, and operational efficiencies—enhancing care quality while building long-term sustainability in rural communities.

The launch of GPCIN builds on more than 40 years of partnership and experience through the Community HealthCare Association of the Dakotas (CHAD). Along with our sister organizations—the Great Plains Health Data Network (GPHDN) and now GPCIN—CHAD provides a coordinated ecosystem of support that includes training and technical assistance, data and technology support, and clinical quality improvement. Over the past five years, for example, our members have adopted a network-wide population health management tool with claims integration functionality.
Together, this ecosystem represents a unique opportunity to align with partners around shared goals of access, quality, and sustainability in primary care. We look forward to new and continued partnerships focused on advancing a stronger rural health system across North Dakota, South Dakota, and Wyoming.
